Chikungunya Arthritis Treatment in Guntur

The fever passes in a week. The joint pain can last months — and when it does, it is treatable rather than something to simply wait out.

What is chikungunya arthritis?

Chikungunya is a mosquito-borne viral infection that causes sudden fever with severe joint pain. In a substantial proportion of patients the joint symptoms outlast the infection, persisting for months and sometimes years as chronic chikungunya arthritis.

The pain is typically symmetrical, affects the small joints of the hands, wrists, ankles and feet, and comes with marked morning stiffness. In some patients it resembles rheumatoid arthritis closely, and a proportion go on to develop an inflammatory arthritis that needs disease-modifying treatment. Distinguishing persistent post-viral arthritis from newly triggered rheumatoid arthritis changes what is prescribed, which is why it is worth assessing properly rather than managing with painkillers indefinitely.

Symptoms to watch for

  • Joint pain persisting weeks or months after a chikungunya illness
  • Symmetrical pain in hands, wrists, ankles, knees and feet
  • Morning stiffness that takes an hour or more to ease
  • Visible joint swelling, particularly of the fingers
  • Difficulty gripping, walking or climbing stairs
  • Marked fatigue and disturbed sleep
  • Relapsing pattern with flares after activity or illness

When to seek urgent care

A single hot, swollen joint with fever needs same-day assessment to exclude joint infection.

How it is diagnosed

Investigation is chosen to answer a specific question rather than as a blanket screen. For chikungunya arthritis that usually means:

  • History of confirmed or clinically diagnosed chikungunya and the pattern since
  • Joint examination and formal joint count
  • Inflammatory markers (ESR, CRP)
  • Rheumatoid factor and anti-CCP to identify those developing rheumatoid arthritis
  • Chikungunya serology where the original diagnosis was not confirmed
  • Ultrasound of affected joints to demonstrate active synovitis
  • Exclusion of other causes of post-viral and inflammatory arthritis

Treatment options at Siva Sakthi

Which of these applies to you depends on what the assessment shows. The options are discussed with you, including what each involves and what recovery looks like.

  • Anti-inflammatory therapyNSAIDs at adequate dose with gastric protection are first-line for persistent post-viral joint pain.
  • Short corticosteroid coursesA tapering course can break a severe, disabling flare where NSAIDs are insufficient.
  • Disease-modifying drugsHydroxychloroquine, methotrexate or sulfasalazine are used where synovitis persists beyond three months or antibodies are positive.
  • Physiotherapy and graded exerciseRange-of-motion and strengthening work prevents the stiffness and deconditioning that follow months of pain.
  • Pain and sleep managementAddressing sleep disturbance and neuropathic components improves function alongside anti-inflammatory treatment.
  • Review and reassessmentRegular review identifies those whose arthritis is evolving into a chronic inflammatory disease that needs long-term treatment.

Common questions

Most people improve within weeks to a few months, but a significant minority have symptoms lasting a year or longer. Persistent swelling rather than pain alone is the feature that warrants rheumatology assessment.

In some patients chikungunya appears to trigger a persistent inflammatory arthritis that behaves like rheumatoid arthritis and responds to the same treatment. Antibody testing and ultrasound help identify them early.

If symptoms have lasted beyond about three months, or joints are visibly swollen, assessment is worthwhile — untreated persistent synovitis can damage joints, and effective treatment exists.
